The Protocols Behind Clean Start Weight Loss®
Clean Start Weight Loss® isn’t a prescription — it’s a structured medical program. Five protocols work together: the GLP-1 medication schedule, adjunct injection therapies, lab monitoring, the nutrition framework, and the maintenance off-ramp. Here’s the clinical detail behind every one of them.
A Structured 5-Visit Protocol
Every Clean Start Weight Loss® patient follows the same structured visit cadence — five touchpoints with their provider across the standard 12-week program. Each visit has a specific clinical purpose.
Full medical history, baseline labs, contraindication screen, individualized protocol designed.
Tolerance review, dose adjustment if needed, side-effect management, hydration check.
Weight and body composition review, dose escalation if appropriate, adjunct therapy review.
Mid-program labs, plateau check, maintenance discussion begins, nutrition framework review.
End-of-program assessment, maintenance protocol walkthrough, taper schedule, guidebook hand-off.
The GLP-1 Medication Schedule
GLP-1 receptor agonists work in three ways: they slow gastric emptying so you feel full longer, they signal fullness to your brain so you stop eating sooner, and they balance blood sugar. Clean Start Weight Loss® providers prescribe one of two molecules based on your medical history.
Semaglutide
A GLP-1 receptor agonist. The active ingredient in Wegovy® and Ozempic®. Generally started at the lowest effective dose and titrated up over several weeks to minimize side effects.
- Once-weekly subcutaneous injection
- Slow dose escalation managed by your provider
- Typical full program runs 12+ weeks
- Compounded versions available through licensed US pharmacies
Tirzepatide
A dual GLP-1 and GIP receptor agonist. The active ingredient in Zepbound® and Mounjaro®. Often produces slightly faster initial weight loss; your provider determines whether it’s the right molecule for you.
- Once-weekly subcutaneous injection
- Multiple dose tiers with structured escalation
- Often selected for higher BMI starting points
- Compounded versions available through licensed US pharmacies
Important: Specific starting doses and the escalation schedule are determined by your Clean Start Weight Loss® provider based on your medical history, current medications, BMI, and how your body responds. This page does not prescribe; your provider does.
The Adjunct Injection Protocol
Three additional injection therapies work alongside your GLP-1 to support energy, fat metabolism, and overall health while you’re rapidly losing weight. These are part of the standard Clean Start Weight Loss® program — not optional add-ons sold à la carte.
A blend of B vitamins (B1, B2, B3, B5, B6, B12) that support energy metabolism, red blood cell production, and nervous system function — all of which can be taxed during rapid weight loss.
Why it matters: Combats fatigue, supports cellular energy.A lipotropic combination that supports your liver’s ability to process and metabolize fat. Lipotropic (“fat-loving”) nutrients help mobilize fat from where it’s stored so your body can burn it for fuel.
Why it matters: Supports fat metabolism during the active-loss phase.Combines L-Carnitine — an amino acid that helps transport fatty acids into your cells’ mitochondria where they’re burned for energy — with vitamin C for immune and connective-tissue support.
Why it matters: Helps convert stored fat into usable energy.Lab Work & Monitoring
Lab work is non-negotiable on the Clean Start Weight Loss® protocol. Your provider runs a baseline panel before any prescription is written, and a follow-up panel mid-program. This is the difference between medical supervision and a website transaction.
| What’s Tested | When | Why It’s on the Panel |
|---|---|---|
| Complete metabolic panel (CMP) | Baseline + Week 8 | Kidney function, liver function, blood glucose, electrolytes. |
| Complete blood count (CBC) | Baseline | Overall blood health, screen for infection or anemia before treatment. |
| HbA1c | Baseline + Week 8 | 3-month blood sugar average; important if you have or are at risk for diabetes. |
| Lipid panel | Baseline + Week 8 | Cholesterol and triglycerides — often improve dramatically during the program. |
| TSH (thyroid) | Baseline | Screen for thyroid dysfunction that can affect weight and energy. |
| Vitamin D, B12 | Baseline (optional follow-up) | Identify any deficiencies that can hamper energy and recovery. |
| Pregnancy screen (when applicable) | Baseline | GLP-1 medications are contraindicated in pregnancy. |
Additional or different tests may be ordered based on your medical history, current medications, or symptoms. Your Clean Start Weight Loss® provider determines your specific panel.
The Ketogenic + Intermittent Fasting Framework
The medication does the metabolic heavy lifting; your nutrition compounds the effect. Clean Start Weight Loss® patients follow a medically-guided ketogenic diet paired with intermittent fasting — a combination clinically shown to lower insulin, stabilize blood sugar, and amplify GLP-1 benefits.
The Ketogenic Side
Approximately 80% healthy fats, 15–20% protein, 0–5% carbohydrates. Your liver converts fat into ketones, which your body burns for energy more cleanly than sugar.
- Lowers insulin by 50%+ on its own
- Reduces hunger hormones
- Stabilizes blood sugar
- Eliminates the sugar cravings that derail most diets
The Intermittent Fasting Side
A 6–8 hour daily eating window. Most patients eat from around noon to 6 or 8 p.m. Fasting overnight (which you already do while sleeping) is extended into the morning.
- Further lowers insulin during the fasting window
- Accelerates entry into ketosis
- Reduces the metabolic stress of constant eating
- Aligns naturally with GLP-1 appetite reduction
The Maintenance Off-Ramp
This is the part most weight loss programs don’t have. The Clean Start Weight Loss® maintenance protocol is a structured, four-stage off-ramp that helps you transition off medication without losing your results. It’s why patients keep the weight off long-term, not just through Week 12.
Around Week 12, you and your provider confirm you’ve hit your target weight and discuss your readiness to transition.
Your provider designs a step-down schedule — typically dose reductions over several weeks, not an abrupt stop.
You move from strict Ketogenic + Intermittent Fasting into a flexible maintenance eating pattern — guided by your Patient Guidebook.
You hold your new weight using the maintenance framework and check back in with your provider as needed.
Your Patient Guidebook includes 20+ pages of maintenance-phase guidance: portion control, the foods that trigger old patterns, how to handle social situations, and how to monitor your weight without obsessing over it.
The difference between a real medical program and a prescription.
Most online GLP-1 services give you a prescription and a phone number. That’s a transaction, not a program. A structured protocol means somebody is watching your labs, adjusting your dose, managing your side effects, and planning your transition off the medication before you ever stop it.
That structure is also what makes Clean Start Weight Loss® repeatable. The same Week 0/2/4/8/12 cadence, the same lab panel, the same adjunct therapy stack, the same maintenance off-ramp — applied to 80,000+ patients since 2011. Outcomes you can predict come from protocols you can follow.
Protocol Questions, Answered
The questions Clean Start Weight Loss® providers hear most often about how the protocol actually works.
Do I get to choose between semaglutide and tirzepatide?
You and your provider make that decision together. Both molecules work; they have slightly different profiles. Your medical history, BMI, current medications, and how your body has responded to other treatments all factor in. Your provider may also start you on one and switch to the other if tolerance becomes an issue.
Is the lab work included in the cost of the program?
This varies by provider and location. Some Clean Start Weight Loss® providers include lab work in their bundled program fee; others bill labs through your insurance or as a separate cash-pay charge. Your provider will explain pricing during your initial consultation. The lab work itself is non-negotiable — it’s how the program stays safe.
What if I can’t tolerate the dose escalation?
You aren’t pushed through the standard schedule. The Week 2 check-in exists precisely to catch tolerance issues early — if you’re struggling, your provider slows the escalation, prescribes anti-nausea medication, or holds you at the current dose for an extra week or two. Some patients reach a comfortable dose lower than the “standard maximum” and stay there.
Are the adjunct injections required, or can I skip them?
They’re a standard part of the Clean Start Weight Loss® protocol. B-Complex supports energy during rapid weight loss; MIC supports fat metabolism in the liver; Lipo-C helps convert stored fat to usable energy. Skipping them is technically possible but means losing meaningful pieces of why the program works. Talk to your provider if you have specific concerns.
Why is the maintenance off-ramp staged instead of just stopping?
Stopping a GLP-1 abruptly tends to bring back appetite and cravings quickly — which is the moment most patients regain weight on other programs. Tapering gives your body time to adjust as the medication leaves your system, gives you time to settle into the maintenance eating pattern, and gives your provider a window to catch problems before they become weight regain.
How long does the whole protocol take?
The active program is roughly 12 weeks — five structured visits over that span. The maintenance off-ramp adds several more weeks of supervised taper. After that, you’re in long-term maintenance, which is open-ended; you and your provider decide how often to check back in. Many patients return for a “tune-up” once or twice a year.
